If you are not able to self-refer to a service, you will need a GP to refer you. Please detail your condition and symptoms here and we will arrange an appointment to discuss.

NHS e-Referral Tracking

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.

To track your e-Referral you will need the following details to log into the e-Referral website:

  • Your date of birth
  • Reference number – this will be on the first page of your letter
  • Password – this will be on the second page of your letter

My Planned Care

This is an online platform which contains average waiting time information for 1st Outpatient Appointment and further Treatment for each speciality under your Hospital Trust. 

 The platform includes helpful advice and information such as how to manage pain, finding mental health support, keeping healthy, and information for accessing financial help and other local support whilst you wait.  It can also help you to prepare for your appointment or operation, including how to form a plan for your care and treatment alongside your healthcare professional.

 Updated weekly, the site is easy-to-use and has ‘open-access’ so carers, friends, relatives and NHS teams can also easily access this information.
