Clinical Pharmacists

Ascent provide in-house Clinical Pharmacists to improve patients access to prescriptions, access to assessments for common conditions and improve medication safety. Each of the Network practices have their own Pharmacist to work in that surgery for their patients. The Pharmacists also have a responsibility to oversee the prescribing safety standards across the Network and work together and with the Multidisciplinary Teams to achieve this. All our Pharmacists are training towards additional qualifications in community care and prescribing, providing our patients with an enhanced access to high quality services. Our Pharmacy team are capable of advising on Blood Pressure and Cholesterol management, advising on alternative medications and over the counter options, streamlining your prescriptions and coordinating medication monitoring necessary for patients to safely continue certain medicines. They may be in touch proactively with our patients to arrange reviews and monitoring, or you may be offered an appointment with one via the Practice administration team, without needing to see a GP first.

Pharmacy Team

Julia Pereira – is our senior pharmacist and pharmacist trainer. She supports Asplands Medical Centre, providing medication reviews, medicines optimisation and aiding with prescription logistics, she can help with “deprescribing” and the processes of coming off certain medications when the time is right. She has a special interest in Cancer Care (Oncology) having worked in this specialty in the hospital environment. 

 

Izabela Kocan – came to us with experience in community pharmacies. She supports Walnut Tree Health Centre providing medication reviews, medicines optimisation and aiding with prescription logistics, she can help with “deprescribing” and the processes of coming off certain medications when the time is right. With her experience in the community pharmacy, Izabela has the advantage of being able to link with her local peers regarding any drug stock issues and available substitutions.

 

Sarah Baker – supports Fishermead Medical Centre providing regular medication reviews, prescription query responses, aiding with deprescribing and the processes of coming off certain medications when the time is right for you.
